The Care Manager promotes a sound therapeutic recovery community. They may oversee a caseload of clients and facilitate groups as needed.


  • Assist residents as needed; respond to client crisis and emergencies as needed.
  • Assist with client Orientation, Assessments, and Intake functions as needed.
  • Monitor medications with clients.
  • Maintain accurate and timely Incident Reports & case notes. Document developments and important events in

accordance with clinical policies for reporting and record keeping.

·  Use Phoenix House systems, including Welligent Electronic Medical Record, to document notes and activities as assigned.

·  Follow program rules and schedules and seeking pre-approval for any unscheduled or unauthorized activities.

·  Utilize appropriate chain of support immediately to report unusual incidents.

  • Provide “On Call” coverage on a rotating basis.
  • Perform other duties as assigned.


  • Demonstrated proficiency in Phoenix House's electronic health record system (Welligent) sufficient to fulfill the

duties required by this position is expected within 30 days of employment.

  • Demonstrate empathy and compassion for clients in groups and daily interactions.
  • Maintain structure and accountability with clients effectively with appropriate frequency and clinical judgment

  • Promote self-help concept within program. Encourage clients to create their own solutions, and encourage

clients to use each other for support.

  • Work in a Trauma-informed manner when interacting with clients
  • Ensure client confidentiality in accordance with all federal, state, and local laws and regulations

EDUCATION/EXPERIENCE:  Bachelor's degree in related field from an accredited institution or equivalent combination of education and experience.

LICENSES/CERTIFICATIONS:  Certification/licensure as required by funding/regulatory sources.

  • Exposure to clients infected with Hepatitis B, HIV, TB, or other Infectious Diseases

SCHEDULING NEEDSEvening, weekend, and/or holiday shifts may be required.
