SUMMARY
Advanced Practitioner in Family Practice, in collaboration with
multi-disciplinary team, functions as a member of the treatment team to deliver
direct patient care to a Substance Use Disorder and Co-occurring patient
population.
ESSENTIAL
JOB RESPONSIBILITIES
- Perform
initial patient screenings and patient history and physicals.
- Provide
assessment, which may include preliminary psychiatric assessments and
pharmacologic management when indicated.
- Collaborate
with referral sources, internal team members and external service providers
to ensure appropriate patient care and continuity of care.
- Initiate,
direct, and modify medical care plan; observe, evaluate and record
behavior, condition and response to medication, treatments and therapeutic
activities.
- Complete
all direct service EMR documentation according to department standards and
timelines.
- Ensure
all clinical care services are provided in compliance with regulatory
standards established by governing bodies.
- Provide
medication and medication injections and routine health screenings that
include taking patients vital signs.
- Pursue
professional development trainings and education that support agency
mission, values, and strategies.
- Demonstrate
use of evidence-based practices.
- Attend
Medical Services staff monthly meetings.
- Ensure
understanding of Evaluation and Management procedures as well as new CPT
coding to ascertain proper billing for patients and appropriate financial
billings.
- Participate
in physician peer reviews as directed by agency Medical Director.
- Other
duties as assigned.

occurs
- Available by phone to respond to and triage medical
issues (true emergencies, call 911).
- Comfort conducting psychiatric assessment plus
- Refer residents for specialty care, as needed
- Strong interest in caring for a diverse patient
population in an inpatient substance abuse setting.
- Experience and interest in substance abuse and
treatment
- Willingness to serve in the role of primary care
provider
- Excellent communication and teamwork skills
- Experience with EMR and Microsoft programs
QUALIFICATIONS

- Minimum experience: one year
providing high-quality health care to a diverse patient population,
preferably within an inpatient setting.
- Strong computer skills, to

- Experience with substance use
disorders preferred
- Bilingual preferred

- Ability to assist client population.
- Ability to work and communicate with
others.
- Ability to serve as a positive role
model to clients.
- Ability to apply organizational
management skills and maintain self and client discipline.
- Ability to prepare and maintain
required documentation.
- Ability to maintain a valid driver's
license.
LICENSES/CERTIFICATIONS
- Current Nurse Practitioner New
Hampshire licensure
- AANP or ANCC certification as a Family
nurse practitioner
- Certified in CPR /ACLS
- Current DEA License with X waiver
